The Office on Aging does not and cannot provide legal advice or counsel and any information on this site is for informational purposes only.  However, the information here should help educate people about some common legal issues relevant to the aging population.

Advance Directives

One of the most popular topics people search for are Living Wills and Health Care Power of Attorney.  These are called Advance Directives because they are to be done in advance.  Here is a good primer on Advance Directives.

There is also a document called the Five Wishes.  This was put together by the organization Aging With Dignity and you can order the forms from their site.  The  Five Wishes is an easy to use legal document that lets your family and doctors know which person you want to make health care decisions for you when you can’t make them, the kind of medical treatment you want or don’t want, how comfortable you want to be, how you want to be treated and what you want your loved ones to know.
